Should your loved one be in the care of either the Dallas or Tarrant County Medical Examiner's Office, please click on the appropriate Medical Examiner button to complete the required release form. Once you have completed the form, please print, sign and fax it to our office at 214-343-4041. Please note that the same person who signs the initial police report must also sign the medical examiner's form.
